胃、十二指肠穿孔缝合术

是溃疡病急性穿孔的手术方法之一。适应证：①穿孔小，瘢痕不多，不伴梗阻及出血者；②腹腔污染严重及不能耐受胃大部切除者。采用硬膜外麻醉，取上腹正中切口。进腹探查证实诊断后，沿胃或十二指肠的纵轴，距穿孔边缘约0.5cm用丝线作全层间断缝合，在穿孔上、中、下各缝一针即可。若穿孔较大，可先用大网膜堵塞穿孔，并将它缝在胃或十二指肠壁上，关腹。一般不放引流。缝合术不但缝闭穿孔，同时要吸净腹腔积液，减少腹腔污染的机会。由于缝合术后，约1/3需再次手术行胃切除术，故只要条件允许，应首先即行胃大部切除治疗。
