Fontan手术
Fontan operation
又称“肺动脉下心室旷置术”。是治疗三尖瓣闭锁、单心室、肺动脉闭锁、多脾症、无脾症等复杂心内畸形的惟一的心内矫治方法。1971年法国学者Fontan首先报道，故命名为“Fontan手术”。手术基本方法是施行右心房-肺动脉转流。仅行右房与肺动脉之间的带瓣管道或直接吻合，称为改良的Fontan手术。Fontan手术适应准则有10条：肺动脉平均压小于2.0kPa(15mmHg)；肺血管阻力小于4wod单位/m2；肺动脉/主动脉直径比大于0.75，且末梢肺动脉发育正常；年龄大于4岁；窦性心律；右房容积正常；心室排血功能要达正常值的60%以上；肺静脉回流正常；无二尖瓣关闭不全；做过体肺分流术者无肺动脉狭窄。术后中心静脉压急骤上升，体静脉系统能否适应耐受，肺循环是否良好是手术成功与否的关键。符合上述准则的病例，术后运动负荷、心肺功能得以改善，远期效果多良好。
