卵巢囊肿蒂扭转
卵巢囊肿蒂扭转是常见的妇科急症。常发生于瘤蒂长、活动度大、中等大小、重心偏向一侧(如囊性畸胎瘤)的囊肿。多在病人体位改变时或妊娠期、产褥期由于子宫位置改变而发生蒂扭转。卵巢囊肿的蒂由骨盆漏斗韧带，输卵管和卵巢固有韧带组成。发生急性蒂扭转后，首先出现静脉回流受阻，引起瘤内高度充血或血管破裂出血，使瘤体急剧增大，一旦动脉循环受阻，血管完全闭塞时则可发生坏死甚至破裂。肿物变为紫色或黑色，周围腹膜发生炎性反应。卵巢囊肿扭转的典型症状为突然发生剧烈腹痛甚至休克，恶心呕吐。妇科检查发现有压痛显著、张力较大的肿块，并有局限性肌紧张。如果蒂扭转发生缓慢，则疼痛较轻，有时扭转能自行复位，疼痛也随之缓解。治疗应行肿瘤切除，术时注意在扭转部位以下的根部钳夹，并切断瘤蒂(夹前不可回复扭转，以免引起栓塞)，因肿物坏死，变脆，很容易破裂，故术时动作要轻。
