自知力
insight

  自知力(insight)亦称“领悟力’,、“内省力”。指病人对自身精神病状态的认识能力。由于病人对精神健康和精神疾病了解的水平不同,很难简单地判定为自知力存在或丧失。最好分别了解四方面的情况:(l)病人能否意识到自己的表现(如过分活跃或欣快)。(2)如果意识到了,是否认识到这些表现是不正常的。(3)如果认识到了,他以为这是精神病还是躯体病或仇人投毒所引起。(4)如果病人承认患病,他是否认为需要治疗。神经症病人多具有完整的自知力,能主动就医,诉说自己的不适,积极配合治疗。精神病患者一般均有程度不同的自知力缺陷,并随病情发展而变化。病人在疾病初期有时尚能觉察到自己精神状态的变化;病情进展后则否认有病,拒绝治疗;病情好转时,自知力随之逐步恢复。自知力丧失在临床上可作为判断精神病的指标之一。自知力的完整程度和其变化,是判断精神病恶化、好转或痊愈的重要根据。 (赵亚忠撰刘协和审)
