多发于肌肉不发达的女性青少年运动员，系髌骨被顶撞向侧方错位时被股骨滑车嵴切割造成，多伴有股四头肌内侧扩张腱撕裂或撕脱性骨折。患膝受伤时即刻疼痛剧烈，随之出现关节积血，关节穿刺之血性液静置后表层有油滴浮游。髌骨侧位及轴位X线摄片可确诊。新鲜骨折如并发髌骨全脱位，可手术切除游离之骨软骨块，把股四头肌内侧扩张腱外移缝合固定于髌骨表面中线附近，而骨折创面可不予处理。陈旧性病例引起疼痛或伸膝功能障碍者，可手术切除髌骨创伤关节面的骨突部分以缓解症状。
