急性化脓性中耳炎或慢性化脓性中耳炎急性发作时，炎症扩展到乳突气房，使黏膜或骨质化脓坏死所致。主要病变为乳突小房积脓。急性化脓性中耳炎恢复期症状未减或加重，仍发热、耳流脓量多，耳后乳突部红肿压痛。X线片示乳突气房混浊，房隔模糊不清或破坏。治疗同急性化脓性中耳炎，重者可行乳突手术。
