Objective:To evaluate the efficacy and safety of high frequency oscillatory ventilation (HFOV) in treatment of neonatal respiratory distress syndrome(NRDS) and neonatal acute respiratory distress syndrome(ARDS).
方法:对1997年1月～2007年6月本科收治的848例早产儿肺透明膜病(NRDS)和248例新生儿急性呼吸窘迫综合征(ARDS)患儿进行临床对照研究,采用HFOV治疗的患儿为病例组(包括NRDS516例,ARDS156例),将给予持续低流量吸氧、面罩吸氧或持续气道正压(CPAP)治疗的患儿列为对照组(包括NRDS332例,ARDS92例)。

新生儿呼吸困难主要表现为呼吸急促、鼻翼扇动、三凹征、呼气性呻吟及异常呼吸，常伴青紫，重者可有肌张力低下，甚至休克。病因有：呼吸系统疾病：①肺部疾病：如肺透明膜病(HMD)、肺炎、肺出血、湿肺等；②气道梗阻：鼻后孔闭锁、喉梗阻、误吸乳汁等；③肺外压迫：膈疝、气胸、纵隔气肿、胸腔积液、血管环压迫、先天性肺淋巴管扩张症、纵隔肿瘤等；④中枢神经系统疾病：如颅内出血、缺氧缺血性脑病、感染、药物中毒、膈肌麻痹等；⑤心血管疾病：先天性心脏病、持续胎儿循环、红细胞增多症等；⑥消化系统疾病：坏死性小肠结肠炎、巨结肠、先天性肠闭锁及肠梗阻等；⑦代谢紊乱：窒息引起的酸中毒、低血糖、低血钙等。即刻处理：吸氧、保暖、保持气道通畅，治疗病因，及时转诊。
