四环素类
本类药物因分子结构中具有共同的氢化骈四苯环而得名，属广谱抗生素，对革兰阳性菌、革兰阴性菌及螺旋体、立克次体、衣原体、支原体及原虫等均有抑制作用。其抗菌机制主要抑制菌体蛋白质合成。本类药物具有酸碱两性，能与酸或碱生成盐。但在碱性水溶液中不稳定，故多制成盐酸盐。国内应用品种有四环素、土霉素、金霉素、强力霉素、甲烯土霉素和米诺环素。前3种系天然品；后3种是人工半合成品，又称新四环素类。金霉素性质不稳定，不良反应多见，只限于眼科外用。常用者为四环素、土霉素、米诺环素及强力霉素等，它们在抗菌谱、临床应用等方面相似，多用于轻症感染，一般不宜用于重症感染性疾病。这些药物口服吸收多不规则，能与肠道中的多价金属离子结合成不被肠道吸收的络合物，故宜空腹服用，避免与抗酸药、钙盐、铁盐、牛奶及食物等同服。吸收有一定限度，每次口服超过500mg后，其吸收并不相应增加。多数药物较少在肝脏代谢。大部分原形经肾脏和胆汁排出，故尿液和胆汁药物浓度较高。酸化尿液可增加四环素类对泌尿道感染的疗效。细菌对本类药物耐药性产生较慢，但一旦产生则常持久不变。天然四环素之间有交叉耐药性，但与半合成品之间交叉耐药性则不明显。属妊娠D类药物，主要有以下不良反应：①对胎儿牙齿及骨骼发育影响；②肝毒性；③出生缺陷。
