急性感染性心内膜炎
旧称"急性细菌性心内膜炎"。由毒力较强的病原体感染所引起的心内膜炎症。病原体以金黄色葡萄球菌为最常见，其次是溶血性链球菌，肺炎球菌、革兰阴性杆菌等均可致病。多在原有瓣膜病、先天性心脏病基础上发生。主要表现毒血症症状，如发热、寒战、贫血、肌肉关节疼痛等，累及心瓣膜后，可发生穿孔，较常出现腱索及乳头肌断裂、心脏杂音变化，早期出现充血性心力衰竭、心脏赘生物脱落，可发生栓塞现象，多见于脑、肺、肾。并可见Roth点和Janeway结、脾肿大，以及血培养阳性，白细胞增高和血沉增快等。本病使用一般剂量抗生素，疗效较差，剂量宜较大，常常联合用药，但易引起耐药。当主动脉瓣膜急性感染一周内，出现二尖瓣组织结构断裂的症象或迅速出现心力衰竭时，可考虑紧急换瓣术。
