十二指肠残端破裂

是Ⅱ式胃部分切除术近期的严重并发症。原因：①十二指肠残端缺血；②残端瘢痕，缝合不严密、愈合不良；③空肠输入襻梗阻，十二指肠内压增高。破裂多发生于术后3～6d，表现为右上腹突然剧痛及腹膜炎体征。破裂在术后48h内，应立即手术，缝合破裂口或十二指肠造瘘。如晚于此时期大多只能充分引流腹腔。
